I'm sorry for the trouble caused to you.
Upon checking your form, I was able to replicate the exact same problem you are having. I did clone your form to figure out why the form is not getting submitted.
There seems to be a glitch where if the "US State Picker" or "Country Picker" is marked as required and then condition is added to show / hide the widget, the form will not submit. It always says "Please wait..".
I am creating a bug report and sending it to our back end team. We will get back to you as soon as we have any update on this.
For now, I will suggest you to please change the widgets to not required and your form should work fine.
