Tables has an extra Tab and when I click on it I cannot get back to the first Tab

In Tables: I can view the Vaccination Status List Tab where data would flow into.

When I click on Exemption for Covid Vaccine in purple, I can no longer go back to the Vaccination Status List and have to close out and go back in?

Also, if I wanted to remove that tab I am not finding a way to do it? We are not planning on collecting exemptions. Just Vaccinations and weekly test results.

We may have to escalate this to our developers for further investigation. But an easier fix is cloning the form. Since there's no submission yet, you will not lose any data. The issue did not seem to persist after I cloned the form.

My concern with Cloning the form is there are 2 forms tied to the original app. If I clone one or both forms will it break the connection to the app?

It will just duplicate the form so you can make modifications to fit a new purpose.

I suggest trying the suggestion of my colleague as it worked when he tried it.

I did what you suggested, cloned the form - made this the active form and deactivated the original one - I still kept it, just deactivated it.

Now, as I figured when I open the App the form I deactivated is no longer available. So cloning the form only resolves the Table issue I was seeing, but because both forms need to be tied to the original app this will not work. Is there a way to tie the cloned form (now active) to the original app?

No, I used the Jotform App template called Vaccine Tracker App. I revised it to match our needs for the 2 selections it already has set up under the schreifelst@district279.org account.

The App creates two forms and can be found under forms to make edits:

1.      Submit for Weekly Testing

2.      Proof of Vaccination

Submit for Weekly Testing is working correctly

Proof of Vaccination is working correctly except in the Table view where we will be reviewing the responses.

If you click on the tables it gives you both of the form spreadsheets which is great ( shown above) but if you click on the Exemption for Covid Vaccine to update the name, there isn’t a way to edit and you are stuck on that tab and cannot toggle only close out:

Next, I tried Cloning the APP (not one of the 2 forms) and now I am seeing promising results:

However, now both forms are completely in their own Table and no longer 2 Tabs on one page – I guess this will be ok as I see I can create charts and graphs and other functionality clicking  +Add Tab to either one.

Something else that I did notice, and I cannot figure out how to change is:

The emailed message that goes to the person who submits the form.

It sends the email but at the bottom of the email it has 4 large “g’s” showing? Any ideas how to remove these from showing?

Also, can you please provide the submission ID of the form that has 4 large “g’s” showing? As you got several forms in your account and we need to test the one that has the “g’s”.
