    Hi,

Thank you for your question. I think a matrix field would best suit your need.

    To add a matrix field:

Click or drag the matrix field tool to your form. It can be found under the Survey Tools section

By default a matrix field has radio buttons selected as the input type.

You can change this by clicking on the Input Type button then choosing the input type you prefer:

Click the Rows and Columns button to edit the labels for the rows and columns

Take a look at this sample form in which I tried to replicate the form that you want to create.

The only problem with a matrix field is that it does not have any auto-calculate function.

In fact, we only have two fields which have a calculate (sum) function. Those are the payment field (paypal, google checkout, authorize.net) and the grading field.

You can probably use a grading field in conjunction with a matrix field. However, it would pose a lot of different problems especially with gathering data. Since if you use two fields, namely a grading field and a matrix field, data gathered would be separate and it would be quite hard to match the numbers entered in the grading field with the description text entered in the matrix field. Not to mention that a matrix field displays submitted data in an entirely different way.

Let us know if you need further assistance with regards to this topic. Thanks!
