Hello, I have been testing your program. I modified a template. I embedded the code into my web site.
Page works fine in I E. But, in Firefox and Chrome, when you click NEXT, it goes to the next page, but about 5 seconds, the SUBMIT button function happens. therefore, only the first page of form get sent to my email.

Upon checking your web page, I was able to replicate the exact same problem you are having. Upon taking a closer look at your web page, I found that you have used your form's source code to embed your form but I did not find that you have added any custom code. Is there any special requirement to use your form's source code?
It seems that there are some code conflict between form scripts and other scripts of your web page. I did check your form's direct URL and it seems to work just fine. Take a look at your form's direct URL: http://www.jotform.us/form/40817658167161
I would suggest you to please embed your form with its iFrame embed code and that should solve your issue. The following guide should help you: http://www.jotform.com/help/148-Getting-the-Form-iFrame-Code
