Form for tracking equipments taken

Hi, I'm interested in making a form of equipment. That each employee can write down what equipment he took and also the quantity .. Let's say he took 2 cameras ... The problem is that I do not see where it is possible to create such an answer that has both an option for the name of equipment and can indicate how much of the same product he took
