    Can a menu be created just like the attached without the Appetizer and other sections? A simpler form with just one scrollable page

    Please note that the "Appetizer", "Soups" & "Salads" section you see are categories added in the PayPal payment field. I would suggest you to please delete the categories from the field or just disable the categories and that will solve your problem. Please check the screenshot below:
