    Hello Christie,

I've forward the form to be checked by our Review team. I'll get back to you as soon as I hear from them. If this form turns out to be a SCAM, I'll delete your submitted data as well.

    Please allow me some time.

    Thank you!

Thank you for bringing this to our notice. The form mentioned seems to be a scam- asking sensitive information. The account has been suspended and the forms in the account are no longer accessible.

Please note that Online Form Builders, including Jotform, are sometimes being used for identity theft and we go to great lengths to prevent this. Our phishing detector will automatically flag the forms found and suspend the account or this can be manually performed by our Staff.

Here are some advices for you to avoid scams:

    • Never fill out a form coming from an anonymous contact, an unknown organization or a company with no website nor legitimate presence.
    • Never give money to get an employment
    • Never provide with critical information such as SSN#, banking info or driver license numbers or pictures to such suspicious contacts.
