Your form was created under a Guest account, which relies on browser cookies to identify the form owner. Once these cookies are cleared or overwritten in any way, you will lose access to your form and its submission entries. To restore access to your form, register for a a free Jotform account using the same email address that you entered in the form's notification email setup.
This will automatically pull over the form and submisisons from the guest account to your newly registered Jotform account. In any case that the forms do not get carried over, you can manually import/clone that form if you know its URL. Click here to learn more about that.

I'm assuming you also lost your username and password. However, I did not find any matches both when I searched for your email address and username (which I assume to be pinnacle12).
