    This form filled in and sent does not show the check box in the mail received.

    Hello ullibrom,

    I'm sorry for the trouble caused to you.

    Upon checking your this form, I did not find any issue with it. Have you solved this problem already? The form should display the checkbox selection in the submission emails.

    Your form's notification email alert displays the correct field names. Please check the screenshot below:

    This problem usually happens when you edit your email template and then add/remove form fields. If you edit email template, any changes you make to your form will not reflect in the email alert and needs to be changed manually.

    The most convenient way to solve this problem is to delete the existing email alert and add a new one which will ensure that all form fields are added in email template. Then you can do all the desired customization in the email template.

    Thanks for your answer.
    I solved this problem already.
    My form's notification email alert displays the correct field names, but the
    form did not display the checkbox selection in the submission emails. So I
    added it manually.
