    Is there a way to make a form that is choice based?

    For example, in the screen shot I attached, based on which drop down item a person selects the form would change to accomodate that selection. The items required for the rest of the form would be different for each drop down option.

    Yes, we can use JotForm's Conditions feature to accomplish this. Here is a short and sweet tutorial about it.
