my account says suspended and once clients click on form it says unavailable

    Your account was suspended due to copyrighted logo.

    Please provide the following information:

    1) The URL where their form is being used/embedded.

    2) Their website or proof of affiliation to a legitimate institution.

    3) Share with us documents that will prove that they are authorized to offer such services and use the said logo.


    Send the information to reviewers@jotform.com with the THREAD #3537950 as the subject. We will handle it from there.

    Ok thank you how long will it take for this submission to be reviewed? And will I be able to have my account back with my form already made and any submissions I may have lost
