Why are customers being asked to sign up when opening up a form I created with Jotform

My site, orangemonkey.co has a form for clients to fill out, however, its asking for permissions and or sign up with facebook, google etc. Is there a way to simply open the page for the customer to start filling it out? Thanks.

I notice the form is "private" and the edit section is unclickable

The form in your account is currently public so anyone with the link should be able to fill it without creating an account.

To make any changes in your form please make sure that you are logged in to your account.
