I don't quite understand why and how I hit my quota to lock me out, when I go to settings and my info/stats are no where near what you say I can't exceed.
Can you please help me understand.

I am sorry for the trouble caused to you. I did check your account and did not find any issue with that. Your account usage is well within the limit of free plan.
I have gone ahead and activated your account again. Your form is back online and should work as expected.
