You are able to upload PDF documents and convert them to forms only if they are fillable PDFs. This means they can not contain plain text, but they need to have fillable fields.

Already done that, but not the same. How can i attach the pdf for you to have a look?

Feel free to send us a screenshot of the PDF you want to use and how does it appear on Jotform.

Please note that when you upload your PDF, Jotform may not entirely copy all the layouts and styling from your original PDF, and some fields may not be detected as well. Unfortunately, if this happens, you can adjust the field connections manually, and try to add custom CSS coding to the Online Form.
