Hi, there are three sections on my form that aren’t showing up when my clients submit answers…. Not sure why? Please help

I was able to confirm that these fields are blank on your form submissions:
Can you confirm if the user intentionally leave these blank? These fields are not required on your form so the submitter can just leave them blank and submit the form:
Please try setting this as REQUIRED so the submitter will be required to fill these fields out before submitting the form.

Hi,
Can you please try enabling the Required field so the user cannot submit the form without this information?
Also, can we ask your permission to allow us to send a test submission directly to your form? We will remove it once we are done testing just for us to assess the issue.

Since you're a HIPAA user we can't log in to your account.
What I did is required the first field of the form and submit a test submission.
can you verify if you received the sample submission and all fields are filled?

Hello Kylie,
To confirm, are you referring to the email notification? Is the email notification not showing all the filled fields in your form? If so, kindly create a new email notification.
