Option to pay with pay pal in addition to paying later (in person)

I'm making an order form for prints and photo products.  I would like clients to be able to choose whether they want to place the order and pay with pay pal or place the order and pay me by check or cash later.  Right now my form just has the pay pal option. 

Also, I've added the products with size and quantity options with prices but I need to have clients be able to enter the file name of the photo they want to use for the product they are purchasing.  How can I do that?  And what if they want to order 1 5x7 print and 1 8x10 print using 2 different photos?  My form as it is now doesn't give them that option.  I couldn't find a template allowing me to do that.  Please help.
