Change App Icon

Hi - I have created an icon and I want to replace the icon that I can then store on the home page of a phone. I have uplloaded the image into my images file but I cannot figure out a way to add the image that I have created onto the phone's home page. If you look on the bottom right of the screen you can see the icon i pulled from the Jotform lists. I have labelled it but cannot add my own image as an icon. Please help... thanks in advance... Please modify the app's icon in the App Builder's Settings. Please refer to the screenshot below:
