Not able to integrate dynamic QR code with my form and email not getting sent to customer after submission

Hello Team,

I was trying to integrate my dynamic qr code with my form, but was unable to do it. I need dynamic qr code which should change whenever the customer puts their name and email id. i saw a link which does exactly the same but didnt get any video of how to do it. Also, once the form is submitted it should go to the respective customer email id (which is also not happening at the moment).
