Can we create a custom HIPAA-omplaint form for a client

Hey good afternoon. I am inquiring in since we have a client we need to make a custom hipaa compliant form for. I know you guys do hipaa compliant forms but are we able to make a custom form or can your team create one? Secondly, are there any booking softwares that you guys work with or have in internal one for scheduling a phone call/appointment that would pop up once the form has been submitted?

You can apply for a HIPAA-enabled account if you sign up for our Silver or Gold plan. In that way, you make custom HIPAA-compliant forms for your clients.
