The issue is most probably related to enabled FTP integration on your form. Please go to the form Submissions page and disable the 'Send Uploads to FTP' option or re-configure it.
Also, your first upload field is configured to accept PDF files and Validate Images at the same time. I think that the 'Validate Image' option needs to be disabled.
