I cannot thank you enough for this wonderful tool! I have created many effective forms for our homeschool co-op. I've created applications, class selection forms and open house reservation forms and they have all worked beautifully! Thank you!
Our co-op is held every monday and we provide hot lunches for our parents and students. We need to be able to create a form that will allow a family to submit their meal choices on a monthly basis. I've created a form for an individual person that works perfectly, but I need to be able to have the entire families' order on one form. Does that make sense? I don't want a family of 8 to submit 8 separate forms. I'd like one form per family.

This form shows the kind of information I need to gather for each person, I would just like to have more than one person's order on a single form.
Any ideas? If there is an easier/better way for me to achieve this form, I am open to ideas. I love JotForm!!

Hello Jonathan and thanks for replying. The option to add people to the form is great, but they would also have to fill in the information about their lunch as well. For example, once the first family member put in their name and then select their lunch selections for each week of the month. But then, how would the person that added their name make their selections?

Is there a way to make a certain selection of fields/questions duplicate and appear conditionally after another name is added?

Yes there is way to make multiple fields appear by using conditions. And that may partially work, please take a look at this form:
It was built following the method on this guide: Create a basic multiple payment form
It apperently will work correctly. However, it will not. To be able to get the submission data of each order form, each one of them will have to be submitted because they are separated forms embedded in another form.
So, I think there is no other way than creating submitting the same order form multiple times.
But if you think this workaround may work for you, I think something you can do is to warn your form submitter to make sure to submit each embedded form so that you can receive the data of all the membes of the family. In that case, please confirm us if you wish to apply this method and we help you to finish your form by giving you the explained instructions.

How abou this..
Using my test form http://form.jotform.me/form/40907071948460 , isn't it there is Quantity also for each of the items/lunch?
You can just provide the quantity for each of the added family member. This way the total order will still match the number of persons availing the lunch.
