How can I make my link tab to open in a new page?

I have a link on my form that takes me to another form, the problem I am having is that the linked form opens on the same browser tab and the applicant forgets and closes the tab thus loosing the information that they need to print and submit. I would like to have the linked form to open in a new tab??

also since the original forms info name and email is filled out can the linked form have name and email prefilled?
