    Hi,

From what I have tested on your form https://www.jotform.com/40895379861168

there is a condition logic assigned to it before you can see the submit button.

You need to select the option YES in the question "Have you been exposed or treated..."

Why isnt the submit button displaying when I preview or publish my form? Image 1 Screenshot 20

Please check also if you have properly setup your form's condition logic. Refer to this guide for more details  https://www.jotform.com/help/57-Smart-Forms-Using-Conditional-Logic

Hope this help. Inform us if you need further assistance.

    Thanks.

    Hello,

I have taken a look at your form and noticed you have a condition attached to the submit button that hides it until the user fills in the firstname text box. You can clear out this condition and your submit button should be visible with the rest of the form as soon as it loads.

    Thanks.
