changing form name

I cloned my check request form and changed the name to expense report form. I have changed the name in every place I can think of...I even customed the link. However, when you go to it online the internet tab still says "check request". How can I fix this?

This is because you need to go to Preferences, and change the page title there at the General Settings tab:
