The URL below is supposed to pass through and prefill some of the fields, but that does not happen.

Yes this no longer works for most of the standard fields like textbox,radio buttons, checkbox and dropdowns. To prepopulate value please use this app http://prepopulate.jotform.io.

I have looked at the service you linked, and it is not useful for what we need. We are prefilling forms with information specific to each user, not one prefill for all users. How would I script this to dynamically populate the form and then automatically generate a unique URL for each set of data? We can look at moving to this model if you can provide the requested information.
However, I do not believe this is the issue, as we have lots of other forms that use the parameterized URL and it works fine. This is one of 4 tickets I have open on the same form, so I firmly believe that there is something fundamentally wrong with this form. Here is an example of another form that works fine:

It seems that your form started working correctly while I was testing your form and answering your inquiry. Did you do any change/fix from your side? It is now populating with no issues:

No, I believe you've got it confused. the last link I provided was a working example to demonstrate that this is not a problem with the parameterized URL functionality in general, just for this specific form. The Arrival form in the original request, is NOT working. The Departure Form you screenshotted is working fine.

You are right - the prepopulation should work as you have posted in the original request - sorry we did not get it the first time.

Should populate the same values after "=" in each corresponding field - but the form does not -

I would ask you to kindly recreate it (not clone it) so that our developers can take a look at the form and figure out why it does not function as it should to prepopulate values from a querystring

Any update on this? I don't see any reason to recreate this, as it will definitely work on the new form. All of our other forms work fine. The departure form (linked previously in this ticket) works well, and is very similar. The only difference I can think of is that the Arrival Form has part of a Spanish translation in it.

This form was reverted back to a previous version as a fix to a different ticket, and now is working. It was related to adding a translation.

Thank you for keeping us posted on this. I am glad that your form is working fine now.
A ticket is already open and our back end team is already looking into this issue. As you have already reverted your form to previous version, I am not sure if our backend team will be able to replicate this issue.
We will also keep you posted on this if there are any updates.
