Whenever my clients hit submit on one of my forms, it goes to a blank page and all the information is lost. How can I fix this?

    Below is our url that is having trouble.  It doesn't happen everytime but it happens enough to be a problem.  Would it help to not have our form embedded on our site but linked to yours? Please help my computer illiteracy.

    Please re-embed the form to your website using its iframe code.

    I am suspecting a script conflict between the form and those in your website because when I tested your form on the website, the date field was not working


    Using the iframe embed should prevent any script conflict.

    Please try this and inform us if issue still remains.
