Why does "You have uploaded an invalid image file type" appear when I upload ANY type of file?

Specficially, this is showing on the official visit checklist form. When trying to upload jpg, word, excel, or any other file, this message appears.

I tried to upload a screen shot to the bottom of this page and I am getting the exact same message!!

Please accept our sincere apologies about this matter! The issue should be fixed now. Please kindly refresh your form. If it still giving the same error after the form reloads, please kindly re-save it to our form builder.

I am haing the same problem I have filled out the same form 4 times now and it's a lenghty process! I don't want to have to reload it and do it again if it's not going to work again! I am trying to upload a PDF and it keeps giving me that message. I am following all the guideline, all lowercase, no spaces..... PDF only. WHAT CAN I DO PLEASE HELP
