I've been tweeking a new form. The emailed results were coming through as a nice little form, however, they are now showing up as text only. I've changed the email settings from text to html and vice versa but it doesn't make any difference. Can you tell me if I checked something in advertently? The nice little graphic is much easier to read.

The only solution to this problem is to re-create your notification email template. To do so, delete the notification first by clicking Setup & Embed > Email Alerts > Notification (Hale Hula Request), then click the Delete Email button in the Compose Email screen.
Next, create a new notification by clicking Setup & Embed > Email Alerts > Add New Email. Configure the new Email to suit your preference.
Of course, do not click the Switch to Text Mode button ever again :)
