Can a database be queried and return data to a submitter of form?

    My company has a MYSQL database that includes usernames and passwords. Our users request passwords and we currently dont have a sound system to provide them with the information and would like a more automated process that a user can submit and be returned the information.

    Can Jot form pull the info from our MYSQL database and send an email/provide it in the thank you page after a submission is completed and certain criteria is met? Security questions answer, user name and email provided.

    Unless my colleagues have additional information on this. You can send MySQL data on to the form first by creating the neccesary PHPs to retrieve the data from your DB to then URL prepopulate  to your form. You can then set the the custom Thank You Pages from the conditional rules:
