    Form Submissions Are Not Reflecting the Correct Form

    I recently made 2 different forms -- a "Creative Brief" and a "Customer Satisfaction Survey". The satisfaction was created as a "clone" of the creative brief. I used the template I created from that one to guide the creation of the survey.

    I've begun to get a couple responses to the satisfaction survey, but 2 issues are coming up:

    - the answers are correct but the questions I see listed are from the creative brief

    - it doesn't seem to show ALL of the responses that clients are filling out

    How do I resolve this issue?

    Hi Pete,

    First of all, thank you for using JotForm.

    I've tried replicating your issue on our end and I've seen that your email notification from both forms is identical. Please refer to the image below. 


    I would suggest you to alter the email notification of your cloned form prior to your liking.
