The Submit button not working on iPhones

The form embedded on your page is actually cut on windows PC. This might appear OK on Mac but not on Windows PC. This is very common when you're using iWeb builder. The easiest way to fix this is to increase the HTML Snippet's height where you have pasted the form embed code. I am not familiar with iWeb but I think you just need to hold the HTML Snippet holder and drag it to increase its height.

Thank you for your answer ... that is what i did already ....
but the "Submit" Button itself is not active on the iPhones but active on iPads ...
can you help me to make it active on iPhones too

I checked the form on your website
but apparently the form is still cut-off and does not display the Submit button
But using the form direct URL http://www.jotform.co/form/32900983417861 presented not issue. The submit button is there and activated.
Can you please try to re-embed the form to your iweb website while using its iframe code embed instead. Use this guide -Getting-the-Form-iFrame-Code
Please get back to us after you have completed this and if issue still remains.

i went according to answers above but...
the "submit" button is visible but only on the iPhone is not active ....!!!!
can you help me

Upon checking your web page, I found that you have not used the iFrame embed code to embed your form. Please use the iFrame embed code as suggested by my colleague and that should solve your problem. You need to just look for HTML snippet on iWeb Builder and paste the following iFrame embed code there:

I am also not familiar with iWeb builder but it seems that you have a <DIV> container and the height of the this div is set as 759px and if you can change it to 859px that should solve your issue. Please check the screenshot below:
