I tried to change my account back to the starter plan and it doesn't show that it has been done. Will you please double check that for me? Thanks!

Since you used paypal in the subscription payment, you wil need to cancel it also at Paypal.
Please follow this steps on how to.
1. Log in to your PayPal account
2. Find the latest payment you have made to JotForm
3. Click on the subscription number for that transaction
4. Click on the "Cancel Subscription" button.
