Hi,
I am looking to make a Likert scale form with columns like Rarely True, Frequently True, etc. The columns would get points depending on which is chosen. It'll consist of 20 questions including 5 categories or labels that will contain 4 questions each. Then I'd show the results with the categories/label and the scores that were accumulated. Hopefully, I explained that well enough but could I do that with your forms?

If you'd like to include submission data in your notification/autoresponder emails, you can simply add those fields to your email content. This feature is available to all plans including free plan.
