We absolutely love JotForm and will be signing up for more than the free account.  We use it for parish registration, currently we have a short form to capture name, phone, email so we can contact for more information.  Can we set up a long form then have folks make a decision long or short when they click on our form??

    Hi,

    Thank you for contacting us.

Yes, what you require can be achieved using JotForm.

Please test this form http://www.jotform.me/form/41006255607447  and inform us if this idea can work for your requirement.

There are several ways to setup a situation for the form to dynamically behave like show more or less info/data as needed.

You can use Page Break field or Collapse fiel (both found in Survey Tools in the builder) to make the form longer/shorter when being used.

Adding condition logic can also enhanced the capability of the form. In my example form, I have applied the condition logic base on this guide

If you are looking for more template form, I also suggest you check the JotForm Template Gallery. You can search for specific form template that matches what you need.

Hope this help. Please inform us if you need more assistance.

    Thanks!
