Form on your site being used as part of a phishing site

    The form at http://form.jotform.com/jsform/12005418611 is being embedded in a page at http://sing-in-education.10001mb.com/ which is being used to capture the logins and password of unsuspecting victim.  Some of our users received the email below with the link to the site that has a jotform form embedded. Please remove this form from your site as soon as possible.
