My form is gone; can I get it back if I upgrade?

I signed into my account and my form is now gone and the link to it says that it reached its maximum number of submissions.  If I upgrade, do I get the form back and can others complete it using the same link?  Also, when I upgrade, it's per month; can I cancel any time?

From my understanding, you are referring to form: http://www.jotform.us/form/40928178530155. It appears that is has indeed exceeded the monthly submission quota. In which case upgrading your account should enable your form back up.
