omit the header on our form?

If you have time could you help guide me to some code that would omit the header on our form?

Also, no matter what CSS we use to align our forms to the left, the forms end up getting cut in half. It's odd.

Also, once a user submits a form, where does it go??? How do we get it to go to our email?

You can delete the header field on the form builder when you edit your form. Is this what you mean here?
