I just upgraded to Premium but my form doesn't appear?

    Can jotform re-enable my form that I had created using the free version?  It's still the same account. I really don't want to spend the time creating another form.

    I can see that your account is now active and set to Premium so your forms should work fine now. If you need further help please post your concern here http://www.jotform.com/contact.
