Can you help create a registration form for a contest?

Asked by Sonja White  on April 14, 2014 at 01:37 PM

I am interested in creating an online form that can be used to register for a contest.  I have questions about how the information is taken from the form and put into a data base.  Also, one of the services you provide is actually creating the form.  And, if so, how the pricing is determined.

I am available by phone at 330-704-0335.

    Thanks so much.

    Hello,

    Thank you for contacting us.

Unfortunately we don't have phone support, the best way you can reach us is through our forum. You can choose from our template section http://www.jotform.com/form-templates/ we have more then 5,000 templates available online. If none of them interests you there's our Form Builder where you can manually create and design your own form.

You can also use custom CSS for styling to meet the design you like.

I have questions about how the information is taken from the form and put into a data base?

You can use MySQL to POST your submission to your PHP page(You need to have a programming skills to do this or you can hire some developer to do this for you). You can follow this guide on how to link your form submission to your MySQL database http://www.jotform.com/help/126-Form-to-MySQL

Also, one of the services you provide is actually creating the form.  And, if so, how the pricing is determined.

Please refer to our pricing page for more information about the packages Jotform offers http://www.jotform.com/pricing
