I am interested in creating an online registration form that can be used to register for a contest. I have questions about how the information is taken from the form and put into a data base. Also, one of the services you provide is actually creating the form. And, if so, how the pricing is determined.

Unfortunately we don't have phone support, the best way you can reach us is through our forum. You can choose from our template section http://www.jotform.com/form-templates/ we have more then 5,000 templates available online. If none of them interests you there's our Form Builder where you can manually create and design your own form.
You can also use custom CSS for styling to meet the design you like.
