I have used your site to create and online form for a tenantcy application.
I spent time creating the perfect form that will work under our legal system, and embeded it on my website.
(this was a few weeks ago)
I have now checked on my site, and the form has reverted to your origional format, Can you explain why?
I am testing it as a free user, if that has any bearing, but so far this is not a good test run, if before we have someone fill in a form, it is not the form we created, but your base form.
Thanks for your time in reviewing my query, I would be greatful for your form creator to work as intended!

This is what I see on your page.
Is this not the form you're referring here? When you say base form, do you mean it restores back to its previous version? This usually occurs when you failed to save the form after making changes in it or the changes might have overwritten when you edit the form on multiple tabs. You can restore your form to its previous version with Revisions History feature.
