I am logged into my account, however I am not able to see any of my forms.

    I checked your amfrazier account and also confirmed that no forms are in this account. I also checked your trash and there was no form either. 

    After further investigation I saw several accounts under the same IP address. Can you please provide us form's or website's url so we can look better into this issue?
