How can I set this registration form sample up for families that have more than one child? Such as address, doctors name and number, phone numbers, and such would be the same for all... and they would pay one amount.
But grade, medical/allergy needs, Camp Activity preference, and such would be per child.

I was able to clone and the form looks great. The only thing is that after the 4th child, there is another child and that one doesn’t have the medical and allergy box; but that is the one that shows up first on the form on site. I know how to delete the boxes below in the medical area; but how do you get that last one to match the other “children” block?

I also noticed that the t-shirt size needs to be with each child. I saw how to duplicate it, but how do you move it up to the other children?

Hello,
After duplicating the field you want to appear in a different section of the form, You can move them by click and hold, and dragging to position as shown below:
