One of my questions can have multiple answers, and currently, only one radio button can be selected at a time for that question.
Also, Is there a way to make the form that is received via e-mail closer to looking like the one that is submitted? I ran a sample submission and the form I get in my in box is just a two collumn, question/answer type form and looks nothing like the original submission form......is there a preference to set to change this?

If you want your respondents to be able to select more than one option, please use the check boxes instead of the radio buttons. The latter is meant for questions that require a single answer only in contrast to the former.
Regarding email notification templates, do you mean you want the exact duplicate of the form as your email template? It can be done but without the radio buttons, check boxes, dropdowns, text fields, etc. Just the static text displaying the answers of your respondents. You can use the Text Mode since it will be easier to modify the template as such, especially if you have little or no HTML skills.
