PDF editor changes, some form answers not populating

I volunteer for a dog rescue and we are having some issues with the form not populating some of the answers.

can you help me troubleshoot this? The questions below are listed individually on the application but come through as below.

1. Current pets in your home. 2. Please put type of Heartworm prevention your dog is on. 3. If you will be a first time dog owner, please explain what you know about heartworms and heartworm prevention. 4. If your current dog is not altered, explain why.

I'm assuming you're referring to the PDF of the form https://www.jotform.com/92111104800135 as I saw this section in the PDF editor. It looks like this was just added in as a text box in your PDF to include it like this. You can remove it from your PDF by going into the PDF editor and removing the section and then including the forms fields as you would like them to appear.
