Form Submission by Two People?

I am trying to understand how to use a form for performance review purposes where an employee would complete the form first and then a manager would follow up and complete other parts of the same form. Is it possible to have two people entering information into the same form?

Thank you -- I think this could work for us, using the Approval comment box where the manager can write their review based on the submission sent in by the employee. (I had hoped to have specific fields in the form where the manager could respond point by point to employee responses, but I can live without that.)

The one thing I can't figure out is how to get the manager's comments to be integral to the form. So when a manager "approves" the form and includes their review in the comment box, how can I have comment be part of a PDF of that same form?

It is possible to include Approval comments. You can find the comment field in the Approval Fields.

Click Settings -> GENERAL -> Customize Approval Request Email

Click Approval Fields -> Approval comment -> SAVE
