BAA help

Hi, I was using the HIPAA upgrade wizard but when I got to the actual form, there was only a sample BAA in pdf form that I could download, but I did not see the electronic form that is shown on the tutorial, where I could input my name, company name, etc. I did not see a "next" or "continue" button at the bottom of the page. What am I missing? Am I supposed to download, print and sign the pdf sample?
