Getting into Jotform help

    Hi! We are not able to get into our team Jotform account for our organization Nefesh - our login is NefeshLA and we're not able to get in. I think we have a former staff member as our backup email so we're not able to receive the recovery info. Thank you!

    Are you unable to log into this account because you have forgotten your password? Email related to this account is l***t@n***s**a.org, are you unable to use this email to reset the password? If you don't have access to this email anymore, we will need the following information regarding the latest Jotform charge so we can verify that you are the owner of it:

    • Amount charged
    • Transaction date
    • Last four digits of your card
    • Expiration date of your card
    • Type of card(Visa, Mastercard, Amex, etc.)

    After you provide this, we will be able to verify your account and update the email of it to the one you are using.

    Thank you for getting back to us. that email is no longer active for us because they no longer work for us.

    We are trying to figure out what company Jotform bills under? We don't see Jotform on our statement.

    Could you make nefeshinfo@gmail.com the primary account holder?

    Hi,

    The account is a paid user. Please provide a screenshot of the last receipt of the paid subscription and the credit card information that my colleague asked for.

    • Amount charged
    • Transaction date
    • Last four digits of your card
    • The expiration date of your card
    • Type of card(Visa, Mastercard, Amex, etc.)
