I am designing a form for my agents to submit their billing to me and the form seems to work as designed. However when I get the email copy some of the form lines have been changed making it unusable for my needs as I need that piece for record keeping.

Please let me know if there is something I am missing.

Here is an example of what I am receiving via email

Sorry about that. To clarify, are you referring to the incorrect labels in the email notification you received as you have highlighted in your screenshot? If so, you can easily fix that when you recreate your email notification. This should generate correct labels and updated fields from your form.

    Here's how:

    1. First, delete your existing email notification. Guide: How to Delete Notifications/Autoresponder

    2. Create a new Email Notification. Guide: How to Setup Email Notification

By the way, I have also escalated this issue to our developers for further investigation so this can be prevented in any form. Upon further testing, I was able to reproduce the problem in the cloned version of your form. When adding new fields and changing its label, the label is not carried over or synced in the email template.

This problem is now fixed. Any changes in the field labels should now automatically reflect on email notifications/autoresponders as usual. If you still have issues with your existing email notification, you can recreate them one more time and it should solve the problem.
