Form Disappears after Reselection in the Contact Menu

In both IE & Chrome, after submitting the form the user is directed to a Thank You page with a Click to Continue link back to the Home page. This works OK, BUT if you then select the Contact menu tab again, when you reach the page, the form has disappeared!!

I was able to replicate this problem. It happens in the following way...

(1) Form is filled in and submitted.

(2) Instead of clicking on Click and Continue the user skips that and just " Clicks on the Contact Tab " in your website menu and the form is gone.

Looking at it more I believe that's a conflict or problem with your website and you would need to reconfigure that. When I checked the browser for errors I found your site has what looks like a custom framebuilder error and because your domain documents don't match since the form has a different domain this seems to have it cancel itself out leaving the spot blank.

Here's the error I see:
