Signature is not showing at the bottom of my form when I use it in PDF format

    Hi, I have downloaded the form to use in PDF format as well as with the QR code and for some reason the signature line is not available on these. Also for my Disclaimer form, when it is signed and sent back I only receive a copy of the signature but I need to receive a copy of the full form with signature. Is there a way I can access this?

    Thanks for reaching out to us!

    I see your form has a fillable offline PDF which doesn't reflect form submissions.

    You can create a PDF document instead that reflects your form submissions.

    Unfortunately that does not seem to help as it takes me to a form I ran as a "test" that I filled my own info out on and will not allow me to change it without upgrading. Is there another way to ensure that the signature portion I have included on the form is on the form for the client. Also, as the second question... is there a way to view the full disclaimer with signature after the client has signed it rather than just the signature after the client has signed? I feel like not being able to produce the full form w/signature would prove problematic in a court situation.

    Hi Faith,

    Can I confirm that do you want to use your form as an online form, and once your clients fill out the form, you will send them a PDF of their responses that include the disclaimer and their signature?

    Or do you want to send them a PDF form that they will out, and send it back to you?

    If you can kindly confirm whether you want to use the online form or the PDF form, we will be able to give you further instructions on how to set up everything correctly.
